Requirements:

The following documents have to be enclosed with your application:

A legalized or notarized copy of:
Valid Passport (as a non-US resident)
Photo Identification Drivers License or National ID
Proof of residence (as water, electric service or similar invoices)

You can have the copy notarized with a notary or advocate or you can go to a consulate to legalize your documents by the public administration of your country.
